2nd North West London integrated kidney care workshop: cardio renal metabolic disease

This workshop is an excellent opportunity for healthcare professionals from various disciplines to network with colleagues, share knowledge and expertise, and learn about the latest research and best practices in kidney care.
The event will feature a range of informative presentations and interactive activities with great focus on cardio-renal-metabolic medicine.
Learning objectives:
- To understand how an integrated care system can support both the clinicians involved in the care of people with CKD and those individuals themselves.
- To gain confidence in screening for and coding of people with CKD.
- To gain confidence in the management and optimisation of people with cardio renal metabolic disorders
- To understand the importance of activation and education of people with kidney disease in order to best ensure optimal outcomes.
